The materials available on this page provide a variety of forms for you, your students, and their families.

If a borrower wants a third party to manage or receive information regarding his or her student loans, we require proper authorization from the borrower, beforehand. This is one of the ways we try to protect a borrower's privacy and prevent financial harm.

  • Information Release Consent form must be in writing, be signed by the borrower, and name at least one person (individual or organization's representative) as the borrower's representative who can receive their student loan information. Note: The Information Release Consent must be accompanied by a Representative's Certification as to the Validity of Information Release Consent if one is not already on file with us.
  • Representative's Certification as to the Validity of Information Release Consent is a required form to accompany each Information Release Consent form. The certification form is to be completed by the borrower's third party being authorized to receive information from Great Lakes.
  • Agent's Certification as to the Validity of Power of Attorney is required to accompany each power of attorney. It is to be completed by the third party being authorized to act on the borrower's behalf.

Great Lakes does not have a standard power of attorney form. Your power of attorney must:

  • Be in writing;
  • Be signed by the borrower;
  • Name at least one person (individual or organization's representative) to act as the borrower's agent;
  • Grant the borrower's agent powers applicable to the borrower's student loan account(s) with Great Lakes;
  • Be properly acknowledged by a notary public; and
  • Comply with the laws of the state in which the power of attorney was executed. (Note: Great Lakes will not provide legal advice on state law execution requirements for powers of attorney.)

We will not accept a power of attorney that does not meet each of these requirements.

If a borrower needs to change how much they're paying each month, a variety of repayment plans are available. Different plans allow them the flexibility of paying less now, but more in the future.

The deferment forms in this section may be shared with borrowers that wish to postpone their payments and believe they qualify for deferment. Refer to each form for specific eligibility requirements.

Note that the forms in this section list Great Lakes as the return address. Borrowers should return deferment forms to their current servicer. If their loans are not serviced by Great Lakes, please advise borrowers to obtain their current servicer's mailing address to return forms.

The forms in this section may be shared with borrowers that wish to put their payments on hold through forbearance. Refer to the form for specific eligibility requirements.

  • Auto Pay Authorization Form is available for borrowers that want to set up automatic withdrawal payments for their student loans serviced by Great Lakes.
